Have you completed your OCF-1?

The OCF-1 is the Application for Accident Benefits form used to start an accident benefits claim after a motor vehicle accident in Ontario. If you have not completed your OCF-1 yet, contact your auto insurance company to confirm what forms are required and where to submit them.
